1.My name is Dudley Chancey. I have been a youth minister and a teacher of youth ministry for many years. I do many youth events around the world that give young people the opportunity to make a commitment to follow Jesus. I want young people to know Jesus Christ. Your voice matters!

Procedures: Your parents have been notified about your youth group participating in this survey. You will be asked a series of demographic questions and questions about faith and values and the practice of faith and values. The survey at no time can be traced back to you. It is totally anonymous so that you can be totally honest with your answers. The survey will take approximately 20 minutes or less to complete.

Eligibility: Must be the ages of 12-20 years of age

Potential Risks: No identifiable risks to participants in this research study.

Potential Benefits: Through your participation I anticipate a better understanding of the current practices of young people concerning their faith and values.

Compensation: There is no compensation for participation in this research, except the gratification that you are providing data that will help you and others know Jesus Christ and his church better.

Confidentiality: You will be totally anonymous in connection with this survey.
