Questionnaire Purpose and Qualifying Statements
We are recruiting individuals medically diagnosed with Beta Thalassemia for a marketing research project on behalf of BluePrint Research Group; a firm that works with health care companies to help these companies better understand the management and treatment of Beta-Thalassemia.
Since we are looking for many different varieties of individuals to participate in this project, I would like to ask you some questions about your diagnosis. If you qualify for the study, you will be compensated (up to US$150.00) for your time in participating in a completed 60-minute interview.
This project is purely concerned with marketing research and there will be no attempts to sell you anything or influence you in any way. All responses are entirely confidential and your identity will not be shared with the study sponsor. We ask that you answer the following questions as openly and freely as possible.
Should you be selected for the interview it will be recorded on audio and/or video (in person interview only) for the purposes of developing a report to represent the opinion of all research participants. Any claims or data contained in any materials shown to you during the discussion should be regarded as hypothetical and some of the information from this study relates to unlicensed indications.
Your responses will be collated with those of other respondents and presented to the sponsor in an aggregate or confidential form. Representatives of the company commissioning the study may be viewing or listening to the research and, in accordance with standard confidentiality guidelines, your identity will remain strictly confidential. The company who is commissioning this research does not hold or see any information regarding respondents.
