Adelphi University-Informed Consent Part 1:

You are being asked to participate in a research study. This is a voluntary participation so we ask that you read the following explanation and ask questions to be sure you understand what your participation means in this study. 

INVESTIGATOR
A student at Adelphi University is conducting this research study in part, for the completion of a doctoral dissertation. The Principle Investigator is Lynn E. Johnson, MS, NP, CNM, RN who can be reached at Lynnjohnson@mail.adelphi.edu. The supervising chairperson from Adelphi University is Patricia Donohue-Porter, PhD.  PURPOSE OF THE RESEARCH STUDY
The purpose of this research study is to gain knowledge about some potential influencing factors related to nursing practice. Nurses and other healthcare providers have been asked to have an evidence-based practice in providing the best care for their patients. This research study seeks to better understand some of the influencing factors related to why nurses do or do not chose to use and evidence-based practice.

DURATION OF PARTICIPATION IN THIS RESEARCH STUDY
If you agree to participate, it will take approximately 40 minutes of your time to complete the survey that is being used to collect data.

PROCEDURE TO BE FOLLOWED DURING THE RESEARCH
If you chose to participate, you will linked to an on-line survey asking about your attitudes and behaviors related to your current organization, your knowledge and experience related to empathy and reflection, and your nursing practice. Completing this questionnaire may lead to some questions about your feelings and your nursing practice but it is not anticipated that the questions will cause any distress. There are several surveys and a socio-demographic form. You may skip an answer to any question or stop answering the surveys at any time because your participation is completely voluntary.

Please add your email contact information in the space provided in the survey if you would like to be entered in to one of two $100.00 Amazon gift card lotteries. You may still enter the lottery even if you have not completed the study.

RISKS OF THE RESEARCH
There are minimal risks for participants related to this study. Every effort will be made to maintain confidentiality and privacy. All on-line surveys will be de-identified therefore; your name will not be linked to this study. There is a limit to the confidentiality that can be guaranteed due to the technology itself. Specifically, although the risk is small, no guarantees can be made regarding the interception of data sent via the Internet by any third parties.

BENEFITS OF THE RESEARCH
While there may not be any direct benefit to you as a participant, the researchers hope that the information learned through this study will support nursing practice and ultimately better patient outcomes.

CONFIDENTIALITY
You have a right to privacy. All information provided by you in this study will remain confidential. Surveys will be coded with numbers by the primary researcher. No other people can access these records. No identifying information will be disclosed without your permission or as required by law. The results of this study may be published in a scientific journal and presented at various scientific or professional meetings.
