This consent form is for the person with diabetes who is identified and discussed in any case studies submitted by their diabetes educator for this competition. 

This consent form is for the purpose of granting of a non-exclusive licence to information and materials included in any case studies submitted for this competition.

By submitting this consent form, you agree to the specified terms and conditions set out below. Please read this form carefully before submitting.

Selected case studies might be published via the following channels as part of the competition:
  • Print publication available to delegates at the Australasian Diabetes Congress (ADC) 2024, previously known as the ADS-ADEA Annual Scientific Meeting 
  • Finalists videos will be posted to the ADEA LMS for viewing and voting
  • Online via the ADEA website available to ADEA members 
  • Oral presentation at the ADC 
  • Publication in ADE, ADEA's official online publication available to ADEA members
ADEA and the sponsor will not use the materials for other purposes beyond this competition. 
ADEA and the sponsor will not use the materials in a manner that may be deemed adverse or defamatory to the person submitting this form.
ADEA and the sponsor will not use the materials for any political or commercial gain.
