Survey to inform strategic planning process

Community Living St. Marys and Area has a long history of supporting individuals and their families in a very individualized manner.  We have a clear vision and principles that direct us in how we act.  We are currently putting together our strategic plan for the next few years and would like to gain input and information from everyone involved with the agency including people supported, family members, front line staff, management group, board of directors and community partners.

As a valued member of our community, we would like to hear from you.

Please take a minute to complete this short survey.  The information that you provide will be held by Sheila Simpson, a third party who is helping us navigate the strategic planning process.  She will take your information, summarize it and add it into the bigger plan which the Board of Directors will use as guidance in determining our specific plans for the future.
