Before you start:

Some of the choices for sponsorship include a foursome:

Premier: $5,000 (2 foursomes)
Major: $3,500 (1 foursome)
Player: $850 (1 foursome)

If you plan to choose one of these sponsorships and also enter a foursome, please have the following items before you begin:

1. Name of each person and their contact information
2. Handicap or average score for each player
3. Each person's clinic preference (sand, chipping, or none)

In order to complete your sponsorship, you may use your credit card or send a check.
