The Active Living Department with the City of Decatur is conducting a Safe Routes to School Survey to assess where students are walking and biking to and from school. Please consider how your child will travel to and from school while completing the survey.

* 1. What school does your child attend?

* 2. How will your child get to school?

* 3. How often will your child walk/bike to school:

* 4. If your child walks or bikes to school, please map their route by following link (https://drive.google.com/open?id=1YXC1kJMd9W_Qufu3olJTwgE72nw&usp=sharing).

INSTRUCTIONS FOR MAPPING: You must have a google account to edit the map. Once signed in, you will be able to click the edit button under the instructions. To add your child's route to the map select the "draw a line" option under the search bar and select "add a line or shape". Click on the map where your child will be his or her walk or bike route and continue to click on along the map to map out their route. When you have reached where your child stops his or her walking or biking route click on the same spot twice to end the route. A box will appear and prompt you to title the route and will have a comment box. Please title the route "Child's School_Grade_Walk/Bike" ("Oakhurst_First_Walk"); providing additional comments about the route is optional.  For a video tutorial - follow the link: vimeo.com/235043220

* 6. Are you interested in any of the following Safe Routes to School activities? (check all that apply)

* 7. If you answered yes to any of the options in the previous question, please provide your name and phone number/email so the Safe Routes Coordinator can contact you about how you can get involved.
