Please complete the registration form to attend the BROC Foundation and Our Lady of the Lake Health 4th Annual Game Day Prep - Free Physical Day. RSVPs will close Friday, April 19th at 5 pm.

Middle School and High School athletes are eligible to participate. All participants must bring their completed LHSAA Physical Form (page 1) with a parent or guardian's signature.

Physicals will be held on Saturday, May 11th from 7 AM until Noon at 8080 Bluebonnet Blvd., Baton Rouge, LA 70810.
