If you or a family member/caregiver looked at the Chemotherapy Education Package on our website, we want your feedback. This survey is for “quality improvement”, which is when we look at what we are doing and see how we can make it better.

The Cancer Centre at Trillium Health Partners will use the information to make chemotherapy education better for patients and families/caregivers. We will be looking at the feedback we get over many months, so that we can make changes in the future.

- your health issue or treatment
