Nomination Form Overview

Thank you for advancing wildfire prevention across the country! With nearly 9 in 10 unwanted, unplanned wildfires caused by humans, Smokey's message remains important to protecting the lands where we work, live, and play. Please review the following information to prepare your nomination package.
Before submitting your nomination, please review the awards guidelines and criteria and take a look through our FAQ one-pager on crafting a successful nomination package.

The form has six sections:
  1. Nominee information: please provide contact information for both the nominee and their supervisor.
  2. Nominator information: please provide your contact information.
  3. Minimum requirements for consideration: ensure your nomination meets the basic requirements of: geographic scope; minimum two years of work; letters of support (gold and silver).
  4. Project narrative: the bulk of the nomination includes three questions to describe the nominee's work. Please be sure to explain how the nominee went above and beyond, any challenges they overcame, new partners engaged, and evidence of reduced wildfire starts.
  5. Additional details: provide additional information that will allow the committee to support the nominee should they be selected to receive an award.
  6. Documentation: option to upload photos and/or letters of support. Gold and silver nominations must have letters of support to be considered for those levels.
You will not be able to save and return to your responses. It is recommended to prepare all of your information, especially the project narrative, in a separate document.
