HSACCC Annual Survey 2024-2025

Thank you for participating in the HSACCC Annual Survey!

In answering these questions, please refer to information from the 2024-2025 Academic/Fiscal Year (i.e., July 1, 2024 to June 30, 2025).

It is recommended you complete a hard copy worksheet of the entire survey for your reference to make it easier to complete the final survey in the live link provided. Although it is possible to complete the survey in increments (saving after each entry), it is recommended that it be entered/completed in one sitting.

The data collected through this annual survey will be used to support the ongoing evaluation and improvement of student health service programs across California Community Colleges. While only aggregate data will be included in the final report and made available to HSACCC members via our website, individual survey responses may be shared with a single outside agency for the purpose of supporting our data analysis efforts only. On occasion, data may also be shared with professional researchers in support of broader college health research initiatives. All data is shared using a reliable and secure platform, such as Dropbox, and access is limited to authorized users of approved research partners. All data is handled in accordance with ethical research standards to ensure confidentiality and responsible use and only deidentified data will be shared out.
