Thank you for your interest in the Bree Collaborative Behavioral Health Integration Initiative (BHII)! We are very excited to partner with primary care clinics and implement our guidelines and recommendations.
We will be selecting 10 primary care clinics for this initial pilot group, providing 1:1 practice transformation coaching beginning January 2020 through June 2021. Please fill out this short survey to give us an understanding of your practice, and current behavioral health efforts.
