Purpose

We know that the ability to share data in a timely way is vital to the delivery of integrated care, but is also one of the most challenging. The attached survey is designed to help you take stock of progress in information sharing and also to help us understand your ambition in this area, your progress in achieving that ambition, and what challenges and barriers are holding you back. It will help us to understand how we can support you and also what we need to do with national partners to help shift some of those barriers and inform the development of national policy. We also want to use it to draw out examples of good practice that we can help share with other areas.

The survey should completed by an individual with a good overall knowledge of your BCF plan and its implementation, and obviously we are looking for a single return from each Health and Wellbeing Board area. While we have sought to minimise technical language in the survey, you may wish to coordinate a response with your information governance, digital or informatics leads.

The online survey will take around 15-20 minutes to complete.

If you have any queries about any aspect of this survey or would like to receive the survey in a different format (e.g. word document) please contact Darren Sugg (Darren.sugg@nhs.net).

We would grateful if the survey could be completed by Monday 13th July 2015 at the latest.

We do appreciate that completing returns such as this is an administrative overhead, but hopefully you will agree that getting a comprehensive national picture of ambition, progress, good practice and barriers will help everyone to collectively make faster progress in this key enabler for delivering integrated care.

Many thanks in advance for your co-operation
