Norovirus Screening Survey

Our objective is to identify tools being utilized by food establishments in the retail and food service industries to manage employee health, specifically Norovirus (common symptoms of norovirus infection include vomiting, diarrhea, and stomach cramping.), or other applications that can be adapted for employee health screening.
 
We would appreciate your organization’s help with our initiative. Please take a few moments to answer the following (3) questions regarding any screening tools your organization may have in place to meet this food code requirement (FDA Food Code Section 2-201.11).
 
All responses will be kept confidential.  The goal of the workgroup is to compile screening tools and create employee health screening best practices to share with retail and foodservice industries to ultimately help reduce the number of Norovirus outbreaks.

The number of employees will help us identify the size of the business; small businesses are usually defined as organizations with fewer than 100 employees; midsize enterprises are those organizations with 100 to 999 employees and larger corporations typically employee more than a 1,000 employees.

The number of locations is also an indicator of size of the business; small businesses are usually defined as organizations with fewer than 20 locations; midsize enterprises are those organizations with 20 - 1,000 locations and larger corporations typically have greater than a 1,000 locations.
