The Root Cause Coalition is pleased to announce the establishment of the Health Justice Award that recognizes and honors organizations that have successfully implemented a program or intervention to reduce health disparities within the past three years. The winner of the Award will receive $25,000 and will be formally recognized at TRCC’s Annual National Summit on the Social Determinants of Health in 2022. TRCC will also produce a highlight video of the recipient’s award winning program and will promote the work of the organization through multiple media channels and outlets.

Four organizations will be selected as finalists, and one of those four will ultimately be selected as the Award recipient.
 
Selection Criteria and Eligibility
  • Organization’s work aligns with TRCC’s mission of collaborating across sectors to improve health equity and achieve health equity
  • Organization engaged members of the community as part of developing or implementing their work
  • Organization’s work demonstrates success to change systems that impact health outcomes relating to one or more SDoH
  • Organization’s work specifically focuses on challenges/needs of groups most affected by health disparities
  • Organization has successfully implemented a program or intervention to reduce health disparities within the past three years
  • The organization must be primarily based in the United States
  • The award may not be given to elected officials or a current Board member’s organization
  • Please be thorough in your responses to the questions below and include all information that you believe is relevant for review of this nomination

Timeline
  • The application for the Award opens Tuesday, October 12, 2021
  • To be considered for the Award, applicants must complete the following form and submit it by 11:59pm PST January 31, 2022
  • The winner will be contacted by TRCC on or before April 1, 2022 to ensure that they can make plans to attend the award ceremony at the Annual Summit in October 2022
