Focus Group Survey 3/14/26

PLEASE READ THE FOLLOWING INFORMATION CAREFULLY

Your answers to the following questions are to help determine whether you qualify to serve on a focus group. Some of the questions are personal in nature, but all information you provide about yourself during this survey and during the focus group is kept strictly CONFIDENTIAL.

Please try to answer all questions fully. You can decline to answer a question, but your refusal to respond may result in us not selecting you for this focus group.

If you are selected, someone will contact you WITHIN 5 BUSINESS DAYS to confirm your availability, and you will be paid $70 for a 3 hour session, when the session is over, in exchange for your participation.

This survey will take approximately 10 minutes to complete. You must AGREE to the questions below, or you will be disqualified from participating in the focus group.
