- Please plan to spend approximately 15 to 20 minutes completing this assessment.
- We suggest that Executive Directors/CEOs or other members of the leadership team complete this questionnaire, although any individual with adequate knowledge of the organization may do so. Only one assessment should be submitted per organization.
- If you want your organization to participate but you don’t have adequate knowledge of the organization, we encourage you to reach out to a member of your leadership team and ask them to complete the assessment. Feel free to ask using this email template!
- Information shared in this assessment will be kept confidential and only published in aggregate, e.g. a chart showing responses from all organizations in a given cause area. Your responses will not be published in connection with your organization.
- At the end of the survey, you'll be asked for the name of your organization, your name, and your email. We ask for this identifying information so that we can share your assessment results and to ensure your organization isn't double counted.
- Each respondent will receive a short customized assessment of their organization's diversity, equity, and inclusion activities. Assessment results will be sent to the email address provided on the last page of the assessment.
