Patient List Validation (FP69/FP22) Impact Survey

Purpose

The LMC Support Network are seeking to understand the scale and impact of recent patient list validation activity, including FP69 and FP22 deductions, across local practices.

The information gathered will help inform discussions with NHS England, commissioners and representative bodies regarding the implementation and impact of the current process.

Responses will be treated confidentially and used to assess the impact of the FP69 patient list validation exercise. Findings may be reported in summary form to support discussions with relevant NHS organisations. Contact details will only be used to clarify responses where necessary.

Please complete this survey by Friday 26th June 2026.

With thanks to Camden LMC for kindly allowing us to use their draft survey.
