<li>Signature and Date</li>
<li>Letter "P" for all passing test scores for the following:

<li>Volunteer Standards of Conduct</li>
<li>Intake/Interview and Quality Review</li>
<li>Basic</li>
<li>Advanced (optional)</li>
</ul>
</li>
</ul>
