Applicant's business must be a current, registered member of the Okotoks & District Chamber of Commerce.  COVID-19 Rapid Testing Kits are for business owner and employee use ONLY.  They can not be used to show proof of a negative test for patrons/customers.

9.An official "Alberta Rapid Screening Tests Application” is required and a signed copy must be returned to the Okotoks & District Chamber of Commerce, to agree to the Terms and Conditions prior to your order being placed. Once your business enters into the agreement, a member of the Okotoks Chamber team will contact your business within 3 working days to complete the remainder of the forms. We will inform you when your tests kits will be available for pick up at the Chamber office located at 14 McRae Street.  You will be responsible for the program at your workplace from there on. Your organization is also responsible for providing reports EVERY MONDAY back to the Okotoks & District Chamber. Failure to do so, and your business will be ineligible to receive additional test kits.
