Informed Consent Form
Overview:
You are being asked to participate in an online survey for a research project titled The Misdiagnosis of ADHD as GAD in Women, developed as a requirement for the completion of a doctoral degree by Jenna Taylor, a clinical psychology doctoral student at National Louis University. The purpose of this study is to investigate the factors that influence a mental health care provider's diagnostic decision-making. Data for this study will be collected from June 2026 to June 2027. The following information outlines the purpose of the study and describes your involvement and rights as a participant.
Description and Expectations of the Study:
You will be asked to read a brief patient vignette and to complete a short questionnaire estimated to take approximately 10-15 minutes. Your participation is voluntary and can be discontinued at any time without penalty. The results of this study may be published or reported at conferences, but participants’ identities will not be revealed. All data will be reported anonymously, no identifying information will be collected, and none of the information that is collected can be connected to any individual participant. All data will be stored on an encrypted USB drive, secured with a lock and key that only the researcher, Jenna Taylor, can access for three years following the completion of the study and then destroyed. After the data collection period is completed, the survey and data will be deleted from the online survey system.
There are no anticipated risks or benefits for this study beyond those experienced in everyday life. The information gathered will enable continued research on diagnostic decision-making and help further our understanding of the components that contribute to a diagnosis. Upon request, after the completion of the study, you may receive a summary of the results and copies of any publications that may occur.
