28th Annual TNP Conference Session S54-Enhancing the Role of the APP Post Stem Cell Transplant

The objectives for this session were: 
1) Autologous and Allogeneic Stem Cell Transplantation (SCT) is a highly complex process used in treating various malignant and non-malignant disease states.
2) It requires intensive care of patients both during their hospital admission and in outpatient follow up. With advances in SCT – patients are living longer however many times left with long-term complications.
3) Integrating patients back into the community setting takes a team approach and APPs will play key roles in providing ongoing post-transplant and survivorship care.
