You are invited to take part in a research study about the effects of artificial food dyes. The purpose of this study is to better understand how these dyes may impact health and well-being. Your participation is completely voluntary, and you may choose to stop at any time.
If you agree to participate, you will be asked to answer questions about your experiences with foods containing artificial dyes and any reactions you may have had. The survey should take less than 30 minutes off your time. Your responses will be kept confidential and used only for research purposes. There are no known risks to participating, but if you feel uncomfortable at any time, you may skip a question or stop participating.

Survey Instructions
Scientists are studying how synthetic food dyes, including Red 40, Yellow 5, Yellow 6, Red 3, Blue 1, Blue 2, Green 3, Orange B, and Citrus Red 2, affect children and adults. Some people have adverse reactions to these dyes, while others do not. This survey helps them learn more about these effects.
Do you or your child experience adverse reactions when you or your child eats synthetic food dyes?
Physical reactions could include: rashes, migraines, seizures, chronic ear infections, etc.
Behavioral reactions could include: hyperactivity, agitation, aggression, intrusive thoughts, etc.

1. If a child under the age of 18 is affected, please fill out the survey for them.
2. If multiple family members react, please submit a separate survey for each person.
