The Parent/Carer Forum has put together a survey for parents and carers to gather information about their experiences and views of SEND services in the East Riding.


Please complete a separate survey for each of your children with SEND.

Responses to this survey will be treated anonymously.
The closing date of the survey is Thursday 17 December.

Completing the survey should only take 10 minutes.
