The following survey has been developed to complement the field-testing versions of the Inter-Agency Case Management Resources. The purpose of the survey is to collect field-user's response to the usability in implementing the tools and resources. Questions will focus on the user's experience and satisfaction. Opportunities to provide specific suggestions and comments are available in each section.

You do not have to give feedback on ALL tools, only ones that are relevant in your context and experience.
