1. Introduction and Consent

Tēnā koutou, welcome, and thank you for being here.

This survey is part of a project to design a Mental Health and Addiction Lived Experience National Representative Body for tangata whaiora and whānau. It is run by Changing Minds with Balance Aotearoa, Te Kete Pounamu, and Thriving Madly. The project is funded by the Ministry of Health.

We want to hear from lots of different people. This includes:
• tāngata whaiora (people who use or have used mental health and addiction services)
• whānau of tāngata whaiora (family member or support person of someone on a mental health or addiction journey)
• people who work in Consumer, Peer Support, Lived Experience (CPSLE) roles
• organisations that work in mental health and addiction.

The survey takes about 20-40 minutes depending on how you answer. Your answers will be kept private. You can stop at any time. You can skip any question you do not want to answer. You must be 16 or older to take part.

Your privacy. All answers are kept private. Your answers will be grouped with others in our reporting. No one will be named. Your email address will only be used to keep you up to date about the project, and to link your answers to a follow-up survey later. It will never be shared outside the project team. You can withdraw your answers at any time by contacting nrb@changingminds.org.nz

Data sovereignty. If you are Māori, your data will be handled in line with Māori data sovereignty principles (Te Mana Raraunga).

We really want to hear from you in your own words.

This survey is about you — what is important to you about a National Representative Body, what would make it work, and what would get in the way. The open-text questions (where you can type your own answer) are where your voice comes through most strongly. Please share what matters to you there.

We also ask some structured questions — things like tick boxes, rating scales, and standard categories. At times these questions can feel limiting, because the choices are fixed. We ask them so we can report on patterns across many people, and so that we can check we are hearing from a wide range of groups. We hope you will bear with us on those.

You are always in charge. Any question can be skipped. Every closed question has an “I’d prefer not to respond” option as the first choice, and almost every list has an “Other” option where you can write your own answer in your own words.
