BenefitBump Provider Network Development Questionnaire

BenefitBump was created to help make the process of navigating employee benefits and time off programs easier for growing families. We also want to help our participants process their stress and anxiety and get connected to helpful resources.

For many health plans and EAPs, a provider search produces results that include all therapists within a certain geographic range, without differentiating their areas of specialization. We'd like to try to address this challenge by identifying providers that spend at least 20-25% of their practice working with growing families. We will use this list for referral purposes.

The survey also asks a few other questions to help us match our participants to a therapist. We apologize if they seem too personal, but we have also heard from families that they may have a preference on who they work with, but don't get that info from most provider search tools. None of these questions are required, but they would be helpful.

We hope that you'll complete this survey and agree to our terms at the end of the document. There is no cost to participate, nor are we trying to negotiate fees. We simply want to make the process easier for participants and steer them to providers that best match their needs.
