Event for Young People with PoTS (Under 18s ONLY)

We hope we may have some additional spaces to offer for our Hampton Court Palace event on Saturday 25th or Sunday 26th April.

We would be grateful if you could complete the following to ensure we have all the information we need and we will let you know ASAP if a space becomes available.

1.What date would you like to attend?(Required.)
2.What is the full name of the young person living with PoTS?(Required.)
3.How old is the young person? Please note that this event is for children with PoTS under the age of 18 only.(Required.)
4.How many parents/carers will be joining the young person? Max of 2(Required.)

10.If allocated a space, please bring a packed lunch and water bottles for your whole group. Other refreshments will be provided including tea, coffee and soft drinks! Alternatively, you can purchase lunch from one of the cafes (serving hot and cold food) at the Palace. There are numerous restaurants within a 10min walk of the grounds also.
11.Due to the limited space available, we will not be able to accommodate everyone in the room we have available. If we have full attendance, we will only have room for the young people with PoTS. Parents/carers/siblings family are welcome to go off and explore the palace and gardens. Families will be welcome to return and join to eat lunch together. We are hoping the weather will be kind so we can use the outdoor space. We do appreciate some young people will not feel comfortable to be left on their own. We want everyone to feel settled and relaxed and will naturally accommodate where we need to.
12.The postcode for Hampton Court Palace is KT8 9AU. There are 10 disabled parking bays available on site available on a first come first serve basis. Parking on site is free for Blue Badge holders but there IS a cost for parking otherwise and this is not included in the ticket.(Required.)
13.We will be taking photos on the day and need your consent to share these on social media/website/promotional content. Please bear in mind this is consent for your whole group.
