Thank you for your participation in this survey, your feedback is important. The purpose of this survey is to explore the experience of pregnancy care patients of Encompass Pregnancy Care Cranbrook (EPCC).The survey will take about 10 minutes to complete.

We would like to hear from individuals who may be currently receiving care or have in the past.

Results will be used to support the Encompass Clinic, and improve the experiences of pregnancy care patients. A similar survey is being done across the East Kootenay so we can share our learnings and understand maternal experiences on a regional level.

This survey is anonymous and your identity will remain completely confidential. Your participation in this survey is completely voluntary. There are open-ended questions in this survey. We encourage you not to provide any identifying information, such as names, in your answers to open ended questions.
