This survey is designed to evaluate your satisfaction with the athletic field rental application and permitting process offered by the Chesapeake Parks, Recreation and Tourism department. As part of our ongoing efforts to continually improve, we ask you to provide feedback to assist the department in assessing existing services and programs.
