and review the 2016 Supplemental Employee Benefits information. This information provides everything you need to know about your available Homeowner's/Auto/Renter's Insurance, Legal Services, Pet Insurance and ID Theft Protection. Being familiar with the information found on this portal will simplify your completing this enrollment form. As always, your Human Resources representatives are available to answer any questions you might have.
Please complete each page of this form accurately and completely by the Open Enrollment Period Deadline of 11:59 PM CDT, Monday, February 22, 2016.
At the end of the Enrollment Form, you will be asked to confirm your decision to submit your enrollment elections TWICE. Remember, if you do not submit an enrollment form prior to the deadline, your elections in these benefits will be considered to be "Waived."
Unlike our medical, dental, vision, life and disability benefits deductions, deductions for Supplemental Benefits are only taken out of the first two pay checks in the month. So in the two months each year where there are three paychecks, no deduction for voluntary benefits will be made from the third pay check. All Supplemental Benefits deductions are made post-tax.
