Research Participant Informed Consent

Please read this consent document carefully before you decide to participate in this research study. This study is being conducted on the behalf of the Nevada Antimicrobial Stewardship Program (NVASP) and the University of Nevada, Reno.

We are interested in your valuable feedback regarding your experiences with antibiotics.  This survey should take no more than ten minutes to complete.  Your participation in this study in completely voluntary and you have the right to refuse participation or can leave the study at any time.  You can skip any questions you do not feel comfortable answering.  There are no foreseeable risks in participating in this study.

If you agree to participate, you will be asked to complete a survey concerning your past and current knowledge, as well as, experience with the prescribing and/or use of antibiotics. You will be asked to provide demographics as well as answer multiple choice and true/false questions.  

The information we gather in this study will be used to create educational modules for Nevada to improve the awareness of antibiotic resistance.  The results of this study are not confidential, but they are anonymous. This means your answers on this survey will not be tied to your person and will not be used against you in any way.
