Instructions for Final Report

Below are five questions that make up the Final Report for Mass Cultual Council's Amplify Grant Program. This program receives a portion of its funding through the NEA and we are required to ask the first three questions as part of this funding. Please select all categories that, by your best estimate, will make up 25% or more of the people involved in the project.
