About this survey

This survey asks important questions about your views of growing older or ageing in your community and will help us understand what are the barriers and increased opportunities for people to ‘age in place’ within their local community.

The target demographic for this survey is people aged over 65 years of age, or Aboriginal or Torres Strait Islander people over 50 years of age, that live in the communities of Beverley, Boddington, Cunderdin, Kojonup, Manjimup, Mingenew, Mullewa, Northampton and Wagin.

The survey is also open to people in this age bracket that may have close ties to these towns, but may not live within the boundaries. For example, if you complete your regular grocery shopping, attend a medical practitioner or participate in other regular social or community activities in one of these towns, we welcome your contribution to this survey.

We expect the survey to take approximately 20 minutes to complete.  We ask that the survey be completed once only. If you would like some help to answer these questions, we can assist you.

There are various ways that you can provide your feedback:

1. complete the survey on paper and return it by pre-paid return post; or

2. complete the survey on-line via Survey Monkey; or

3. register your interest in the survey by telephoning 08 6389 4500 and we can:

       a) call you back to complete the survey by telephone; or

       b) let you know when and where you can attend one of the upcoming face to face community sessions in your area.

You may also see posters about this survey in your community, such as at the Shire office, Community Resource Centre or library, which will also tell you when and where you can attend one of the upcoming face to face community sessions in your area.

Rural Health West has been engaged by the WA Country Health Service to collect responses to this survey. We will collect a series of personal data, but not your names and as such, your responses cannot be traced back to you in any way. We will use your answers to help us plan and evaluate services in your area, learn more about what’s happening in your community and to improve how we collect information. 

The information we collect may be provided to other agencies (for example to your local Shire) which may also assist them to understand and plan for your needs.  Any information released will not be able to be traced back to you in any way.
