Introduction

Thank you for participating in the Chronic Pain Illnesses National Health Insurance Survey for the National Fibromyalgia & Chronic Pain Association (NFCPA).

This is your opportunity to make your voice count in a way that will inform policy decisions for millions of people affected by chronic pain illnesses.  Your participation will help us to better understand your concerns, needs, and barriers in accessing affordable and appropriate healthcare.  This detailed survey will guide NFCPA advocacy at National Institutes of Health for future comparative effectiveness research to support insurance coverage.

Participation in this survey is completely voluntary.  Anonymity is protected, and no identifying information is collected.  The survey permits only one response per computer.  

PURPOSE OF THE SURVEY
The purpose of this survey is to understand the healthcare needs of Americans with chronic pain illnesses and to then educate members of Congress, and federal and state policymakers. The U.S. Congress intends to repeal the Affordable Care Act (also known as Obamacare) and replace it with new healthcare legislation.  By filling out this survey, you are also helping the National Fibromyalgia & Chronic Pain Association develop a position paper regarding the rights to treatment for people in pain, ensuring their dignity and honor.  By providing detail that represents your lived experience with (a) chronic pain illness(es) and accessing necessary healthcare, accurate education and advocacy can be developed.

Thank you very much for your participation.  In the third quarter of 2017, the NFCPA will post results and make statistics available for your use.  That will be unidentifiable and grouped in aggregate.
