Adult Day Parent Satisfaction Survey 2022-2023

Please rate your level of satisfaction regarding the following topic areas as they relate to Melmark New England’s servicing of you and your adult. Some statements will require you to rate your level of satisfaction based on your general impressions, your observations, your interaction with personnel and/or the progress of your adult. Statements have been grouped into the following topic areas: General information, ISP Process, Communication, Health Care Services, and Quality of Life indicators. Your comments, as well as your rating, would be most appreciated. We are viewing this satisfaction survey as a means of assessing the areas where program development is strong and where it needs improvement. Your accurate assessment, including constructive comments, is necessary to promote quality program development. There may be some statements that are not applicable to you and your adult and therefore please make this notification.
