We respect your confidentiality and do not require you to provide for personal or identifying information. If you choose to provide your contact information, an authorized staff person may follow up to gather more information on how to address your comments or any barriers you may have identified.
How will this information be used?
Your responses will be used to generate a report that will be reviewed by the Management Team and the Board of Directors and inform our annual Accessibility Plan.
