The purpose of this survey is to provide doctors, cannabis companies and law makers important information to further our mission in providing consumer protection and reliable data to support medical patients. We need your help to do that and we are asking for a few minutes of your time in filling out this survey. Please fill out this survey after the patient has taken the tincture for 10-15 days (Complementary bottle should be 10-15 days worth). The survey consists of approximately (15) questions.

Please try to be detailed in your responses where appropriate. If you do not have an answer or do not feel comfortable answering a question, please state "N/A" or "do not wish to discuss" or "uncomfortable" or the like, before moving on. We will never reveal anybody's identity or personal information without their consent, so if you wish to leave your family's name out of the survey, please indicate that where appropriate.

Thank you in advance for being so generous with your time. We are tremendously grateful for your help and feedback. Please contact for any technical assistance, questions or concerns.

We eagerly await your valuable input!

Thank you,

Calm CBD
The herbs in the Calm formula offer support for the nervous system and may aid in the relief or reduction of many symptoms associated with stress and anxiety.  They will be more effective when combined with a nourishing and appropriate diet.  They may be used to help provide relief from symptoms of hyperactivity and ADHD, tremors, palsy, and seizures by calming the nervous system and nourishing the endocannabinoid system in the body.  Keep in mind that every individual’s body is unique and may respond differently.  It is best to use small doses of 10-15 drops to start and the patient can increase their dose incrementally until the proper dose is found.  It is important to wait at least 2 hours after the first dose to see results before taking more tincture. Best used with fatty foods.

Knowing how much tincture to take can be confusing, but it doesn’t have to be.  Herbs will affect everyone in slightly different ways.  Therefore, we can’t give general dosing instructions that work for everybody.  The following tips should help you find your perfect dose within a relatively short period of time.  Keep in mind that your required dose may change over time.

Drop=individual drops     Dropperful=filling the dropper

 Start small:
We recommend initially starting with 5 drops.  If you feel mild effects, but not exactly what you want, try increasing by 5 drop increments.  If you don’t feel anything at this dose, you may want to jump up to ½ a dropperful.
Delayed onset: Taking tincture is different from smoking in that it can take 1 to 2 hours before you feel the full effects.  Keep this in mind when choosing when to take it and when considering taking more.

Be Patient:
It may take several tries before you find the dose that works best for you.

Be consistent:
Depending on your condition, the formula may work best for you if taken moderately 2‑4 times a day rather than in one dose.

If you experience unpleasant effects, stop taking immediately.

Thank you in advance for being so generous with your time. We are tremendously grateful for your help and feedback.
