ASCRS Video Mentorship Series - Video Submission Form

Author and Video Qualifications Page

Please review the Video Submission Guidelines prior to completing the online form.

Video Requirements
  • Videos must be 2 – 10 minutes in length of continuous uncut footage of the session topic unless editing permissions are otherwise noted for the video topic.
  • Videos should not have audio (no music, no narration, etc.)
  • Videos must be stripped of direct identifiers. Videos cannot contain:
    • Protected health information (PHI)
    • Full face recording
    • Unique identifying marks
  • Videos cannot be date stamped (indicating the date of service)

2.Video Consent Confirmation: Authors must confirm that consent for video recording was obtained. If consent was not obtained, video is ineligible.(Required.)
3.Author Acknowledgement of Use: If my submission is selected, I understand by submitting my video here I am granting ASCRS and its permitted successors and assigns, and those acting with ASCRS's authority and permission, the unrestricted right and permission to use the final Video Mentorship webinar recording (which may include my video) and any dirivative works thereof for online education purposes.(Required.)
