APPEAR Augmented Reality Study Screening Pt. 1

Pre-Screening Disclosure Statement

The purpose of this screening interview is to see if you meet the criteria for taking part in our Assessment of Psychological and Physiological Effects of Augmented Reality (APPEAR).  This questionnaire will take approximately 5 minutes.  In a moment, you will be presented with a list of questions.  You may choose not to answer these questions.  You also may choose to stop participating in this questionnaire at any time; if you want to stop, please exit the survey without submitting.  
 
Responses to these questions will be kept confidential and stored securely. By agreeing to take part in this research screening procedure, you give us permission to collect your phone number and/or email address so that we can confirm your scheduled study appointment time, and contact you for study follow up. You can choose if you want, or do not want, to take part in this research screening procedure – it is up to you.  If you refuse to answer the questions or stop answering them at any time, there will be no penalty. However, if you do not answer all of the questions, you will not qualify to participate in the research study. 
 
The risk to taking part in this questionnaire is very small.  The screening questionnaire is not designed to ask you for sensitive personal information, but it is possible that some people may feel uncomfortable answering these questions.  If you qualify to take part in the study and are interested in taking part, then you will be directed to an online scheduling portal which will record your name and contact information; this will be kept confidential.  If you are not interested in the study, then any personal information you may have provided will be destroyed. 
 
Participation in this study should be an enjoyable experience, as you will have the opportunity to experience immersive content and provide feedback on your experience.  You may also acquire useful lifesaving skills through the augmented reality Combat Lifesaver (CLS) training. The results obtained are expected to assist Technology Leaders in evaluating the effects and potential benefits of training in an augmented reality environment.  
 
You will not be paid for answering questions in this questionnaire since it is only to see whether you qualify to take part in the study.
