Our records show that you were a leader in the NCHS Club for your campus. Please take a few minutes to complete the following annual renewal form to let us know your club's status, new contact info, and to answer a few questions about the mental health awareness of your campus.
**It is very important to distribute the Parent Letter before you begin filling out this form. Please complete this form no later than September 30th so we can be sure to include your club in our outreach and opportunities for this year.
