Incident Survey

The survey is for AMETFO to monitor the incidents which are occurring in members' classrooms. With it, we are tracking the implementation of PPM 144/145 (Ministry of Education's Policy/Program Memorandum - Safe Schools), as well as implementation of policy regarding Health and Safety. Please complete this survey each and every time you experience a violent incident. An AMETFO Released Officer may contact you as a follow-up.
