Office of Alzheimer's and Dementia Care Inventory

Aspen Solutions has been contracted with the New Mexico Aging and Long-Term Services Department’s (ALTSD) Office of Alzheimer’s and Dementia Care to conduct an inventory of organizations serving people living with dementia and/or their personal caregivers/care partners.

The information gathered in this survey will be used to compile a directory of services and inform the ALTSD’s strategic plan for Alzheimer’s disease and other dementias. Other dementias include Parkinson's disease dementia, other Lewy body dementias, frontotemporal dementia, vascular dementia, mixed dementia, etc. The survey will take you about ten minutes to complete. By completing this survey, you consent to participate in an inventory of dementia-related services in New Mexico. Your participation is voluntary. You may skip any questions you prefer not to answer.
