This is a survey to be filled out by representatives of organizations located in Alaska that serve seniors/elders. Only one representative from each organization should fill it out. Your answers will help shape the State Plan for Senior Services, which guides funding and programming in Alaska. This survey is voluntary, and you can skip any questions you do not feel comfortable answering. You are not required to put the name of your organization on the survey, and your responses will be confidential. There is no risk or direct benefit to you or your organization for filling out this survey. The indirect benefit is that when your responses are combined with the responses from other organizations, the Alaska Commission on Aging will get useful information to guide the development of the State Plan for Senior Services.
