1. Teacher Evaluation Form

Dear Educator,

Thank you for taking the time to evaluate The Arts Council's Wells Fargo Arts-In-Education Grantees for the 2015-2016 year.

The Arts Council supports arts education programs that reach more than 43,000 students each year. The arts have been proven to lower the dropout rate and transform the lives of young people. Exposure to the arts can improve self-esteem and problem solving skills as well as foster creative development and appreciation for the diversity of our community. Students enriched with arts education score higher on standardized tests and go on to bring creativity and diverse thinking into the workplace.

This year the Arts-In-Education programs are supported by Wells Fargo, the Winston-Salem/Forsyth County Schools along with your donations during the Annual Arts Council Campaign.

If you have any questions about The Arts Council's Arts-in-Education Grants please contact Dara Silver, Director of Member Services and Grants at dsilver@intothearts.org or (336) 747-1426.

This survey is currently open and located online at https://www.surveymonkey.com/r/AIE15-16. It will close June 10,2016.
