AFS Tournament Overage ID Check Form

This form is to be filled out by Team Officials only for the submission of an ID Check on a suspected overage player in an AFS Tournament including Macca's City Cup, Shepparton Cup and VicFutsal Cup. A $50 fee will apply to submit the request which must be paid at the bottom of this form.

The process is as follows:
- Upon successful submission of the form an AFS official will check the ID of the requested player.
- If the player is found to be of the correct age then the fee is forfeited.
- If the player is found to be overage or cannot prove their age under the Overage Player Policy in the Rules of Competition then the money will be returned. Rules of Competition can be found here
