Before submitting a plan, we encourage all organizations to complete a self-assessment.

A Word Document of the form is available for download to help you draft your plan prior to beginning your online submission. We strongly encourage you to draft your plan in a separate document then cut and paste it into this online form.

Once you begin your plan in this online form you may quit and return. However, you MUST use the same computer and browser to return to your plan. To save your information YOU MUST COMPLETE THE PAGE you're on and click "Save/Next". Incomplete pages will not be saved when you return to this site.

A final copy of your plan will be emailed to you in the form of a PDF and to the offices of New Jersey State Council on the Arts. If we have any questions you will be contacted. All plans will be reviewed in April, 2021.
