On March 9, 2020, ADAA made the difficult decision to cancel the 2020 Annual Conference due to the global health crisis surrounding the COVID-19 virus.  ADAA will incur a significant financial hit for cancelling but the well being of our attendees, exhibitors, sponsors, staff, and supporters is paramount. We hope you will please consider donating all or a portion of your registration fees to help ADAA offset our losses.

Through this survey, you will be able to:

1) Kindly donate all or a portion of your registration fees to support ADAA's professional educational mission
2) Defer your registration fee to the 2021 Annual Conference in Boston, MA (March 18-21, 2021)
3) Request a full refund

This survey must be completed by April 30. If you do not respond to this survey by April 30 your registration will not be eligible for a refund and will automatically be deferred to the 2021 conference. Please make sure you complete each field. Incomplete survey responses will result in processing delays.
