Dear Colleague,
Our research team is conducting a survey to better understand health professionals’ knowledge, attitudes and practice behaviours regarding advance care planning.
We are inviting respiratory, practice and palliative care nurses, physiotherapists, occupational therapists and social workers to complete the below survey, this should take approximately 15 minutes to complete. The survey will take you to only those questions relevant to your discipline.
This survey will be carried out in the UK, Australia and New Zealand. This research builds on studies undertaken by our research team looking at respiratory consultants (Australia & New Zealand) and community nurses’ (UK) views on advance care planning in patients with chronic respiratory disease.
Submitting your completed questionnaire is an indication of your consent to participate in the study and to our results being published in a form that does not identify you. You can withdraw your responses any time before you have submitted the questionnaire. Once you have submitted it, your responses cannot be withdrawn because they are anonymous and therefore we will not be able to tell which one is yours.
