Thank you for participating in the 2014 Agency Study Survey!
Your input is vital to the success of the Agency Study so please be assured that your answers will remain completely confidential.
The Agency Study Survey will take between 10 to 20 minutes to complete and is divided into seven sections. We will ask you questions about the following topics:
* Client demographics and services
* Food Programs
* Resources and Capacity
* Nutrition and Wellness
* Staff and Leadership
* Strategy and Goals
* Partnership with ACFB
If you need to consult other key staff or volunteers for the answers to some of these questions please do so. We have designed the survey so that it can be saved and returned to.
Instructions for saving the survey:
• Your answers will be be saved every time you click the “Next” button at the bottom of the page.
• You may return to a completed section of the survey at any time to change your answers by clicking the "Previous" button at the bottom of the page.
• When you are ready to resume or finish the survey use the same link in your survey invitation email to access the survey. You will be returned to the uncompleted portion of the survey allowing you to navigate the survey as outlined above.
Once all survey sections have been completed be sure to click the "Done" button on the final page to submit your answers.
